Annotation | In the late 1960s in San Francisco a task force of concerned citizens was set up to identify better ways to meet the needs of older people in the area. As a result, in 1971, they founded On Lok Senior Health Services, a community-based outpatient "continuum of care" service, which included all the medical, health and supportive services needed for older people to remain in their homes. Since then, On Lok has evolved to become a provider of comprehensive medical, rehabilitative and supportive services for the older population. This guidebook forms part of a training programme (together with a video) which aims to provide long-term care providers with an understanding of the On Lok service delivery model and the role of the multidisciplinary team in co-ordinating care for frail older people. It covers: the care co-ordination process; the team process; the facilitators role; and service decision-making issues. [EPICS collection]. |
