The natural history of delirium in older hospitalized patients
 — a syndrome of heterogeneity

AnnotationParticipants in this United States survey of a hospital's inpatient surgical and medical wards were screened daily for confusion, using the Diagnostic and Statistical Manual of Mental Disorders (DSM) III-R criteria. The Delirium Rating Scale (DRS) was used to describe clinical characteristics of delirium. 15% of patients over 65 developed delirium during their hospital stay. Over two-thirds had delirium on a single day, but delirium may occur at any time during a hospital stay - with considerable variation in its presentation and time-course. The most common causes were medications, metabolic abnormality and infection. Clinicians and researchers need to consider the great heterogeneity of delirium in their studies and when caring for patients.
