Questionable dementia: clinical course and predictors of outcome

AnnotationThe term "questionable dementia" is used in this study to describe a broad group of older people with cognitive impairment who do not meet the criteria for dementia at initial evaluation. The study examined the clinical features of such patients at a memory disorder clinic, and assessed the use of specific clinical features and neuropsychological test measures in predicting who met criteria for dementia, or Alzheimer's disease, at follow-up between 1 and 2 years later. At the final follow-up, 41.3% met the criteria for dementia, 44% were rated as non-demented, and 14.7% remained as uncertain. The study found that poor performance on the modified Mini-Mental State (mMMS) delayed recall item may be a useful predictor of the diagnosis of dementia and Alzheimer's disease. Further development of potential predictors of diagnostic outcome in patients with "questionable dementia" is suggested.
