Annotation | This is the fourth in a series of six articles discussing imminent reforms in primary care. The new white papers on primary care present opportunities for general practices to extend and develop their services to patients. This article discusses some examples to illustrate some of the possibilities for joint working: a practice wishing to develop a service for people with learning disabilities; a community trust contributing to extended primary care in the inner city; and a hospital at home scheme. The authors point to the potential for pooling health and social service budgets. |
