Annotation | This article describes a pharmacist's assessment of medication, health and functional status of 49 older people living at home, in support of a decision for pharmacists to be members of multidisciplinary teams providing home care. Pharmacists can identify drug dosage and misuse problems which would otherwise lead to loss of independent living status, hospital admissions, and death. |
