AnnotationA literature survey and audit by census questionnaire were used to examine the context of and structures for care planning in long term residential care for older people in Scotland. Subjects covered included, for context: aspects of wider infrastructure of care; small group living; establishment aims and objectives; decision making forums; keyworker schemes; and supervision of staff. Structures concerning care planning arrangements included: methods of individual care planning and forums where these were discussed; care management; care planning networks; recording needs; roles undertaken in relation to care planning; review of care plans and the care planning system. Case studies examined the process of care planning, decision making, goal setting relationships, and relationship networks, roles and systems. The census findings show the high incidence of structures for care planning. Recommendations are made to increase the overall effectiveness of systems for care planning.
