| Annotation | The focus of this study is to provide an evaluation of the different forms, types and models of care management which have emerged for older people and people with mental health problems. This article describes the three phases of research to be undertaken: mapping the developing pattern of care management, to provide a broad national picture of different forms; obtaining detailed information on and delineating the models identified; and a comparative evaluation of these models. |
