Annotation | This report sets out the findings from an exploratory project undertaken by the Social Services Inspectorate (SSI) to examine the assessment of older people with dementia who need support to live in the community, focusing on health needs and multidisciplinary work. The project undertook visits to five local authorities which were known to be developing multidisciplinary arrangements with primary and community health care staff, and held discussions with social services and health care staff. The report discusses the key issues raised, and provides examples of good practice. Action checklists are included, which are intended for those in contact with, or assessing, older people with dementia. |
