Annotation | This gives an outline of the Darlington Community Care Project, one of 28 pilot projects funded under the Government's Community Care programme, which provided an alternative to long-stay hospital care for frail elderly people. An evaluation of the Project confirms that, by providing a home care service managed by a case manager, older people requiring both health and social care can be cared for successfully in their own homes.
