AnnotationThis annual report summarises the work carried out by the Health Service Ombudsman (Commissioner): summaries of completed investigations into complaints against the NHS are to be found in separate regular reports of "Completed investigations". Title changes have occurred throughout the history of this publication reflecting the varying regional areas of operation and services covered. (After 1990-91 report, the Ombudsman covered England, Scotland and Wales; since 1999 the report is based only on the NHS in England but where appropriate, comparative data for Scotland and Wales are included). Also since 1999, the title of Ombudsman rather than the previous Health Service Commissioner has been used, following a new structure that was set up after a review of the three main public sector ombudsmen schemes in England. Since 1998/99 report, two additional areas of investigation are carried out: the actions of family health service practitioners (GPs), and, the exercise of clinical judgement. During 2002-03 the Ombudsman received a significant increase in the number of complaints, of which 1,300 were about continuing care in the NHS. This followed the publication of a special report, NHS Funding for Long Term Care (HC 399), by the Ombudsman's Office. Continued as: Parliamentary and Health Service Ombudsman from 2004-05. (KJ).
