Annotation | This is a special issue about the Community Care Program, a foster care programme for frail ageing patients discharged from Johns Hopkins Hospital, Baltimore, MD, and developed by the hospital's Social Work Department. The Program was funded by the Robert Wood Johnson Foundation, and its aim was to demonstrate and evaluate the provision of foster care to frail, ageing patients who would otherwise have been discharged to nursing homes. Articles provide background to the development of foster care for older people, and describe and evaluate the Community Care Program's quality of care. (RH). |
