Annotation | According to the Hospital Advisory Service, the "swap" system - the method of exchanging one place in a residential care home for one in a hospital - is still operating in many areas. The author suggests that assessment of older people's medical and social needs is the first step to overcoming this problem. The community also has to learn to care. (RH). |
