AnnotationPublic Health England commissioned the Housing LIN (Learning and Improvement Network)to produce this briefing. Drawing on the Housing LIN's knowledge of the sector, it looks at the importance of end of life care delivered at home. It considers the context, inequalities in end of life care, and examples of good or emerging practice. It is intended to be a practical guide for those working in mainstream and/or specialist housing, care and support, and public health to understand their respective roles, and how they may work with each other and with the wider health care system, to help people to have their end of life care wishes met.
