| Annotation | Nottingham City is one of six areas in the country that has been selected by NHS England as a Vanguard site to develop a new model of enhanced healthcare in care homes. This article describes the vision for the Vanguard programme's outcomes based model that brings together health and social care services with rehabilitation and which meets the complex medical needs of care home residents.
