Annotation | Joint health and social care budgets are to become a reality: £3.8 billion has been set aside for this scheme from 2014/15. This article notes that the government spending review in June 2013 aimed to encourage a seamless discharge process between health and social care. The authors present some findings regarding the use of a London Health Needs Assessment (LHNA) and other tools for assessing a patient's needs, and the usefulness of such needs assessments on discharge from hospital.
