AnnotationThe authors describe a new tool to guide assessments and reviews with people with dementia. Inspired by the Recovery Star developed by the Mental Health Providers Forum, the 'My Outcomes' tool works by combining narrative testimony and rating scores to generate outcomes. The tool consists of semi-structured interviews, an action plan and a rating scale based on six domains: meaning and purpose; relationships and independence; choice and control; hope and self-esteem; and stability and consistency. The aim is to evaluate the quality of life of people with dementia; and collecting the information in this way can establish the effectiveness of interventions, help to prepare for the future, improve outcomes over time.
