AnnotationDischarge from an older people's mental health service presents many challenges both to patients and mental health professionals. This pilot study considers how social inclusion and approaches to recovery can be used within older people's services to help facilitate planning that meets an older person's complex needs post discharge. The article notes three main aims of the group in promoting an older person's full participation in chosen social and leisure pursuits. These were to enable group members to: prepare psychologically and practically for discharge from the day hospital and/or mental health services; identify and access community groups and activities in their locality; and promote understanding of the benefits of attending community groups. Having looked at outcome measures and evaluations, and reflected on the process of running the group, the authors conclude, following an independent review process, that the pilot project appeared to have been a success.
