Annotation | This guidance aims to help carers and professionals caring for someone with dementia to open up conversations about end of life wishes and preferences. It is based on conversations with around fifty people affected by dementia, people with dementia, carers and former carers. The booklet also suggests other useful resources, and is part of a range of materials published as part of NCPC's role as lead organisation of the Dying Matters Coalition, which aims to raise awareness of dying, death and bereavement.
