Annotation | The aim of this literature review is to provide an overview of the integrated care approach, which is focused on the needs of clients and involves services planned, managed and delivered by a range of co-operating organisations, professionals and carers. The article explores integrated care as a concept, its aims and relevance, levels of integrated care practice, essential elements of integrated care, implementation and tools for implementing and developing integrated care, and the dynamics of integrated care. The author argues that there is no consensus about a set of relevant elements for integrated care and that a generic evidence-based quality management model is needed for integrated care improvement. The article concludes by posing three research questions about integrated care, and notes that these will be addressed in an article in a later issue of Journal of Integrated Care. (JL). |
