AnnotationThis is the first report of the National audit of intermediate care. It covers patients discharged from intermediate care services during 2011/12, and includes organisational level data for the period 2011/12 and, for comparison, 2010/11. It defines intermediate care as "a range of integrated services to promote faster recovery from illness, prevent unnecessary acute hospital admission and premature admission to long-term residential care, support timely discharge from hospital, and maximise independent living." The audit is a partnership project between the British Geriatrics Society (BGS), the Association of Directors of Adult Social Services (ADASS), AGILE (Chartered Physiotherapists working with older people), the College of Occupational Therapists - Specialist Section Older People, the Royal College of Physicians (RCP), the Royal College of Nursing (RCN), and the NHS Benchmarking Network. The audit highlights a wide variation in service models being used nationally, with differences in the extent of multi-agency integration, the scale of services provided, and how intermediate care fits in within a locality's full range of health and community services.
