Annotation | This is a proposed sector-wide partnership agreement to moving forward with personalisation and community-based support. It draws on learning from implementing Putting People First across England in the past three years, and focuses on areas where further action is required. The agreement underlines the necessary connection between preventative, community-based approaches and personalised care and support; and provides a general framework for action.
