AnnotationSerious case reviews (SCRs) are conducted to learn from past experience; to improve future practice by acting on the learning; to improve multi-agency working; and to review adult safeguarding procedures. The Department of Health (DH) commissioned this study to explore specific aspects of adult SCRs in England and Wales. The research team conducted a national survey and interviewed 14 people with experience of commissioning and conducting SCRs. This report presents findings from the national survey regarding the purpose, conduct and protocols of SCRs. (RH).
