AnnotationPersonalised transition is a system for enabling young people with complex needs to leave school and to achieve active citizenship within their communities. Personalised transition was developed at Talbot Special School in Sheffield in collaboration with the City Council, NHS Sheffield and the Learning and Skills Council (LSC). The model draws heavily on the experience of using person-centred planning, and has four key elements: family leadership, curriculum for citizenship, coordinated expert support, and individual budgets (IBs). The model is fully described and published in a report by Alison Cowen (with additional material and editing by Simon Duffy and Pippa Murray) from the Centre for Welfare Reform in association with IBK Initiatives (see website, www.centreforwelfarereform.org).
