Annotation | Intermediate care enables older people to have access to a range of intermediate care services at home or in designated settings. This article outlines developments in health and social policy since the inception of intermediate care. It presents three case studies that explore the positive contribution that mental health staff have made to the intermediate care team in South Gloucestershire. A multiprofessional team approach enables staff to respond to the changing situations and needs of clients and carers. (RH). |
