Annotation"Frail older people" are defined as patients who are usually aged over 75 - and often over 85 - with multiple diseases (which may include dementia). This document aims to help local health communities and organisations to improve the quality and value of care for frail older people, using urinary tract infections (UTIs) as a tracker condition. It is one of a series of documents produced by the Delivering Quality and Value Team at the NHS Institute for Innovation and Improvement as part of the high volume Healthcare Resource Groups (HRG) programme. It used a literature review of the recognised evidence of delivering optimised care, and a thorough analysis of data from Hospital Episode Statistics (HES). It. covers the key characteristics of example local health and social care organisations providing high quality care and value for money, and indicates measures for improvement. (RH)
