Annotation | Farleigh Hospice set up the Hospice Outreach Project (HOP) following loss of premises rented from the local primary care trust (PCT) in Braintree. This article describes how a mobile hospice is taking end-of-life care into the community in Essex, providing information, counselling, support, a point of access, and most recently, bereavement support groups. HOP has made the Hospice more "visible", resulting in a 15% increase in general practitioner (GP) referrals. (RH). |
