AnnotationThis 'Dementia services guide' is underpinned by the concept of personalisation, which means that people with dementia and their carers (formal or informal) will be empowered to shape their own lives and the services they receive. An introductory section briefly explains the different forms of dementia, its prevalence in the UK and London, projections of future need, and spending on dementia and its impact on primary care. Section 2, the integrated care pathway, is based on recommendations made in the National Dementia Strategy, 'Living well with dementia' (2009). It provides guidance about effective services, in respect of patient experience, workforce competencies and outcomes. Section 3, general hospital care pathway, relates to Objective 8 of the Strategy, improved quality of care for people with dementia in general hospitals, and identifying leadership for dementia in general hospitals, defining the care pathway for dementia there, and the commissioning of specialist liaison older people's mental health teams to work in general hospitals. Section 4, memory services, provides guidance for commissioners; and Westminster Primary Care Trust's draft commissioning specification for memory services is used as an example. (RH).
