AnnotationThere is growing realisation across the care spectrum that effective support for vulnerable people can be delivered only when services are truly integrated. This Society Guardian supplement aims to draw out some of the key themes of the Managing New Realities Conference (held in Birmingham, March 2004), supported by the Disabilities Trust, Housing 21, OLM and Turning Point. Among themes covered are: extra care; dealing with an ageing population; a rehabilitation service provided by collaboration between voluntary and statutory sectors; a rapid response nursing team; mental health services; and integrating care services.
