Annotation | This article examines a project set up in 2004 to promote the health and well-being of people aged over 75 living in Caerphilly, south Wales. The multidisciplinary project offered holistic, nurse-led screening in people's homes. It resulted in reduced admissions to hospital for fractured femur, pneumonia and stroke during the period 2004 to 2006. (KJ/RH). |
