AnnotationThe Reablement Service in North Devon is a problem solving service comprising interdisciplinary assessments, goal setting and rehabilitation programmes for referred people aged over 16. The service is provided mainly in rural areas, has an open referral system allowing individuals, carers and healthcare professionals to refer directly, using a categorised priority rating. Assessment and treatment take place mainly in the person's own home. The main aims are: to promote and maintain independence to frail, vulnerable people; avoid unnecessary admissions to hospital or long-term care; and to facilitate earlier and effective discharge. Following work with the Audit Commission (which featured as an example of good practice in the Commission's report, 'The way to go home'), the Reablement Service has evaluated its work in the light of proposals set in the National Service Framework for Older People (NSF). This report brings together material on how the service was established and has evolved, and. includes quantitative analysis and qualitative review. (RH)
