Annotation | The study aims to describe current advance care planning practice (ACP) in care homes for older people, to what extent this is currently undertaken, how this is done, and to highlight good practice already in use, in respect to planning for care when communication or cognition becomes impaired, and/or at the end of life. The authors report on Phase 1, a postal questionnaire survey sent to 500 managers of care homes (213 responded, a 42% response rate). Managers viewed ACP as a positive and beneficial process which they recommend residents complete. Most care homes had staff trained in communication and listening skills, palliative care and bereavement care. Fewer have staff trained in ACP, religious practices or spiritual care. Managers reported lower levels of confidence regarding their knowledge of end of life issues and supporting staff to undertake decisions. They also reported staff confidence as one of the biggest barriers to consultation with residents about end of life care; also challenges in finding out and carrying out a resident's wishes. Good ACP practice comprised attention to ensuring that ACP was focused on the individual and tailored to their needs and abilities. (RH). |
