AnnotationThe care pathway is a multidisciplinary, multi-agency, planned approach to care delivery, the elements of which are presented as guidance notes accompanied by flowcharts and an overview model of best practice. The pathway elements comprise: aid to recognition of dementia; meeting the needs of carers; managing behaviour that challenges; moving from one environment to another; management of cognitive enhancing medication; and discharge pathway from acute hospital care. Appendices provide more detailed practice guidance on referral to specialist mental health team; recognition and treatment of depression; discharge from care; person centred care; and NICE guidance. Also included are the Folstein Mini-Mental State Examination (MMSE); Geriatric Depression Scale 4 and 15 item versions (GDS-4 and GDS-15); the 6 item Cognitive Impairment Test (6CIT); and Carer Strain Index.
