Annotation | Willow Housing and Care, a housing association in Wembley, north-west London, is helping older people make the transition from hospital care back to independent living. This article describes how timely discharge from hospital improves joint working between housing, health and social services, and provides support and advice to older patients enabling them to lead fuller lives, play an active part in decisions, and be involved in social and recreational activities.
