Annotation | First published by PEPAR Publications in 2000, "The health and social care divide" is intended to provide a human face to those research findings relating to the problematic nature of the health and social care divide. The book includes case studies which are composites compiled from the experiences of people whose lives have been severely affected, often for the worse, by the issues discussed. The problems covered relate to hospital discharge, rehabilitation, intermediate care, continuing care, domiciliary care, mental health problems, and primary care. The book explains why partnerships are important and what helps or hinders partnership working; reviews the current policy and legislative framework; summarises research findings; and provides good practice guidelines. The book is designed as an introductory text for those working or training to work in a multi-agency environment in health and social care. (RH). |
