Annotation | This study highlights the use of short screening tools for primary care assessment of older people at high risk of hospital admission, institutionalisation or death. The authors give an overview of the advantages and disadvantages of six such assessment tools. In future, standardised assessment systems in primary care could generate individual risk profiles, by inclusion of some of the risk factor items from these short screening instruments. (RH). |
