Medicaid home and community-based services for the elderly
 — trends in programs and policies

AnnotationExpenditure and participation trends for three Medicare home and community-based services (HCBS) programmes serving older people (home health, waivers and personal care services - PCS) are presented for 1990-2002. This article also reports a national survey of policies and on these programmes in 2002. Although the trend data show a rise in waiver and PCS programmes and participation, they also reveal falling expenditure per participant for PCS programmes, declining annual growth in total HCBS expenditure, and large inter-state variations in waivers. The use of cost control policies such as spending caps and large waiver waiting lists in many states contribute to the gap between demand and supply for Medicaid HCBS for older people. (RH).
