Financial management services in consumer-directed programs

AnnotationShifting from an agency-based model of personal assistance services (PAS, also known as home care) to consumer direction has important consequences for both recipients and workers. In consumer direction, recipients assume the responsibilities of employing their attendants - for both self-directing their supportive services and being responsible for fiscal matters. Many states have ceased these fiscal responsibilities for recipients in publicly financed personal care programmes, by using Financial Management Services (FMS, also known as fiscal intermediaries). This article introduces the major types of FMS organisations used by Medicaid consumer-directed personal care programmes, and examines the extent to which the varied approaches can and do serve the needs of both recipients and workers. Despite the expansion of consumer-directed programmes and the accompanying emergence of FMS, these organisations have not been studied of evaluated extensively. The paper discusses the challenges, opportunities and policy implications of current practice. (RH).
