Annotation | The Ile-de-France region was very seriously affected by the 2003 heatwave. This retrospective cohort study of deaths that occurred between 1 and 20 August 2003 was conducted in five departments in the Paris area (Essonne, Seine et Marne, Seine Saint Denis, Val d'Oise and Yvelines) among beneficiaries of the Allocation personalisée d'autonomie (APA), a stipend specifically allocated to dependent subjects aged 60+. Their dependency level was determined by the GIR (groupes iso-resources, defined by French law) used to fix the APA amount. Subjects' GIR group classification and demographic variables were obtained from departmental administrative files. Among the 31603 beneficiaries alive on 31 July 2003, 16779 lived in the community and 14824 in institutions. Between 1-20 August, 858 subject died: 300 community dwellers and 558 institutionalised (mortality rates of 2.7%, 1.8% and 3.8% respectively). Independent risk factors for mortality were: for community dwellers, age, sex, and GIR group; and for institutionalised older people, age, GIR group and living in a region highly exposed to heatwave mortality. Independent risk factors for the sample overall were age, sex, GIR group, type of residence (institution/community), living in a region highly exposed to heatwave mortality, and income. The dependency level was associated with mortality during the 2003 in France, especially for older community dwellers. Dependency might help identify high risk subjects and guide targeted prevention measures against heatwave-associated mortality. (RH). |
