Annotation | Hospital discharge and continuing health care is one of the sub-studies of a research project on inter-agency collaboration funded by the Department of Health (DH). This Update reports its findings on policy issues, 'blocked beds', rehabilitation, and joint working with social services. The main inter-agency collaboration study examines whether the framework of strengthened incentives and clearer responsibilities for joint working set out in the 1989 "Caring for people" White Paper is, in fact, facilitating collaboration. It also explores whether it is enabling the development of integrated health and social care services focused on the needs of individual users and carers. (RH). |
