Annotation | Providing Integrated Health and Social Care for Older Persons - Issues, Problems and Solutions (PROCARE) is a project of the European Union (EU) Fifth Framework Programme (Quality of Life and Management of Living Resources, Area "The Ageing Population and Disabilities"). PROCARE aims to help in defining the new concept of an integrated health and social care for older people in need of care, by comparing and evaluating different modes of care delivery. This volume draws on the project's second phase (2003-2004) - empirical fieldwork and a cross-national analysis of models ways of working in the nine participating EU Member States: Austria, Denmark, Finland, France, Germany, Greece, Italy, the Netherlands and the UK. Integrated care is examined from the perspectives of stakeholders, service users and carers. Integrated care and inter-professional working, the role of the family, working conditions, long-term care services and access to provision are also investigated. (RH). |
