AnnotationThe LinkAge Plus Programme is based on partnership working. It aims to: build an evidence base to support the case for joined up services in terms of delivering better outcomes for older people; test the limits of holistic working; and build a body of good practice and lessons learned from other communities and partnerships. This document summarises proposals for the eight local authority based pilots in Devon, Gateshead, Gloucestershire, Lancaster District, Leeds, Nottinghamshire, Salford, and Tower Hamlets. Each pilot focuses on local needs and how best to integrate services within those locations. The Programme's context relates to the Government strategy, "Opportunity age: meeting the challenge of ageing in the 21st century" (Cm 6466; TSO, 2005) and "A sure start in later life" (Social Exclusion Unit, 2006).
