Derivation and confirmation of scales measuring medical directors' attitudes about the hospitalization of nursing home residents

AnnotationThe authors surveyed 446 nursing home medical directors about the necessity of admitting residents to hospital for eight clinical conditions, comparing the ratings to those obtained from an expert panel, to derive a relative hospitalisation score. The survey (79% response rate) was part of a quality indicator project organised by the Center for Health Systems Research and Analysis (CHSRA) at the University of Wisconsin. The relative hospitalisation score demonstrated that medical directors were slightly less likely to recommend hospital admission than expert panel physicians. Factor analyses yielded 10 scales focusing on nursing home functioning, economics, resident-specific considerations, and physician attitudes. Eight of the 10 scales and significant bivariate associations with the relative hospitalisation score, and 6 had significant multivariable associations. Hospital admission decisions are complex and involve clinical and non-clinical factors, so that medical directors identify multiple determinants across several domains in their decisions to admit residents to hospital.
