AnnotationCase management for frail older people aims to combine both preventive and responsive care for patients at high risk of deterioration in their health. Case management is also a key component of the government's Community Matron policy. Evercare is a form of case management which was introduced in nine pilot Primary Care Trusts (PCTs) by UnitedHealth Europe in 2003; and this evaluation outlines the main findings of PPCRDC's evaluation of Evercare carried out between 2003 and 2005. NPCRDC found that the Evercare approach to case management provided additional contacts, monitoring and treatment options that were highly valued by patients and their carers. However, although there were individual examples of avoided admissions, there was no overall effect on emergency hospital admissions.
