Annotation | A report from the Nuffield Institute for Health, University of Leeds was commissioned to look at care facilities for older people across former Avon County. In response to the report, the Mental Health In-Reach Team (MHIRT) at Callington Road Hospital, Brislington, Bristol was set up as a one-year project providing advisory and clinical support in assessment and care planning to residential homes to help them with residents' mental health needs. This article outlines the types of support offered, the emphasis on promoting person-centred care (in line with the National Service Framework for Older People, NSF), and the communication links established between the MHIRT and the community mental health team (CMHT). The hope is that the service is continued within Bristol and that the in reach team model is transferred to other mental health services. (RH). |
