Annotation | The Preferred Place of Care document originated in Lancashire, and was developed as a tool to record patient and carer choice. It is patient-held and is intended to be used for a a variety of different health and social care settings. Although the document appears to provide a unique insight in to patient and carer experience and fulfil a unique function in terms of empowerment, there is, to date, little evidence available on its effectiveness. This paper reports on the management and outcome of a pilot of the Preferred Place of Care document in south Essex. Here, its use by an end-of-life palliative care support initiative demonstrated its worth in empowering patients, relatives and staff. (RH). |
