Annotation | This report provides an update on progress on implementation of the White Paper, "Our health, our care, our say : a new direction for community services" (Cm 6737, January 2006), and sets out the next stages. It includes a road map to implementation, which is intended to be a resource to support key actions for local implementation. It outlines progress against White Paper's goals, using such examples as: the Partnerships for Older People's Projects (POPP); individual budgets; and an end-of-life care strategy (citing the work of Marie Curie Cancer Care and the Palliative Care Co-ordination Centre). One short case study included is Dudley PCT's "The Right Care and the Right Place" for people with long-term conditions; while Sandwell Community Caring Trust is given as an example with regard to changes in the health and social care system.
