AnnotationMedication error is a leading cause of death in Americans aged 65+. The identification of errors and the development of cost-effective models are the main focus of articles in this issue of Home Health Care Services Quarterly, which the Haworth Press has simultaneously published as a monographic separate. Issues of medications management in home care settings and in transitional care from hospital back to the community are considered. The articles report on: a project to reduce falls and improve medication management; integration of a medication management model (MMM) into outcome-based quality improvement in a pilot for a rural home healthcare agency; the MMM as an experiential education tool for pharmacy students; the Prescription Intervention and Lifelong Learning (PILL) Program and reaching the housebound; pharmacist interventions; polypharmacy and possible drug-drug interactions among diabetic patients receiving home health care services; improving post-hospital home medication management; and risk of medication errors at hospital discharge and barriers to problem resolution. The final article asks what can be achieved in the international community regarding medications management for older people. (RH).
