| Annotation | This is a 4-page introductory document to the revised Care Record Guarantee which was first published in 2005, and which is to be regularly reviewed following feedback to the Care Record Development Board. The rights and responsibilities of patients and NHS staff are highlighted in this introduction. The National Information Governance Board for Health and Social Care (website: www.connectingforhealth.nhs.uk/nigb) has taken over the functions of the former Care Record Development Board (CRDB) which closed at the end of September 2007.
