Recognising heart failure in elderly patients with stable chronic obstructive pulmonary disease in primary care

Annotation51 primary care practices in the Netherlands participated in this study of 1,186 patients aged 65+ with chronic obstructive pulmonary disease (COPD) diagnosed by their general practitioner (GP) who did not have a diagnosis of heart failure confirmed by a cardiologist. 405 patients underwent systematic diagnostic investigation, which resulted in 83 receiving a new diagnosis of concomitant heart failure. Independent clinical variables for concomitant heart failure were a history of ischaemic heart disease, high body mass index (BMI), laterally displaced apex beat, and raised heart rate. The addition of natriuretic peptide measurements and electrocardiography further increase the accuracy of the diagnosis. (RH).
