Annotation | This resource, funded by the Mental Health Foundation, has been developed over four years at the University of the West of England, with the aim of informing nursing care to people with dementia in general hospital settings. The resource is based on the findings of a research project that explored how people with dementia experienced admission to a general hospital setting. It attempts to encourage nursing staff to 'get to know' the person with dementia they are caring for, in order to improve the stay in hospital. The resource has been designed to inform practice, and each of its six elements is designed to be reproduced and distributed throughout hospital wards, as required. The resource comprises: a report, detailing the research and development behind the resource; an A2 poster to be used as a teaching resource and displayed in ward settings; an A4 copy of the A2 poster; a blank copy of the individualised information gathering sheet; an example of a completed information gathering sheet, detailing a fictitious patient; and a 'quick-guide' that can be given to nursing staff. The resource is available as a PDF document, downloadable from the Change Agent Team (CAT) website. The author's 2003 PhD thesis, "Acute nursing care for people with dementia: what happens when a person with dementia is admitted to hospital for acute care?" formed the basis for the resource.
